The Rainbow Valley inter-professional team offers the following services to the community:

• Chronic Disease Monitoring & Education
• Smoking Cessation
• Laboratory Services
• Social Work Services

Our social workers offer various group programs throughout the year. As well, individual social work counseling services are available to adults, couples and families residing in the Rainbow Valley area.

External providers offering additional services to the community at the Rainbow Valley site include:

• Community Lung Health Program
• Home Care Coordinator (Ontario Health at Home)
• Diebetes Education
